Mission & Philosophy |

Our mission is “To empower and enrich the lives of all people served.” We believe clients and their families must be able to choose the outcomes and services that meet the goals they have set. Our responsibility is to assist the clients in obtaining the supports to achieve the outcomes they have chosen. The Center provides services based upon a Person Centered Plan (PCP) developed with the client, their family, service professionals and consultants. Comprehensive assessments are used as a basis. The PCP addresses the 25 Outcomes and Supports from The Council on Quality & Leadership in Supports for People with Disabilities. The PCP is specific to the clients and their selection of the Outcomes and Supports most meaningful to them. Family members are active participants in all planning. The Center considers for placement, clients who are unable in their current setting, to receive the services they require. We are unique in providing services to both ambulatory and non-ambulatory clients of all ages. Young people who have been placed in the Children’s Program are eligible to continue their placement in our Adult Program. This opportunity addresses the necessity for continuity, which all of us desire. The Center has been providing award winning services to the Northern Illinois area, as well as the entire state, for over eighty-five years. Although we focus upon the service requirements of clients from the northern Illinois area, we also have clients from throughout the state. The Center provides community based services in both small community settings and a residential facility. Children in the residential facility and community settings attend schools in their community. Adults in the Community Integrated Living Arrangement (CILA) Program attend the Center’s Day Program, Job Training Program, or Supported Employment Program. |
